Ministry Agreement
Please read this agreement before requesting ministry. You sign it on the Request Ministry form by typing your full name, and it applies to every ministry session with DOL Freedom Ministries, including the Bloodline Deliverance Intensive.
About this agreement
This agreement is between you and DOL Freedom Ministries ("the ministry", "we", "us"). By signing, you acknowledge that you have read, understood, and agree to the terms and conditions below.
If you have a question about any part of it, email [email protected] before you sign.
1. Nature of service
Ministry sessions, including the Bloodline Deliverance Intensive, are faith-based, spiritual ministry sessions focused on prayer, deliverance, and biblical guidance.
This service is not:
- professional counseling
- psychological therapy
- medical treatment
- licensed mental health care
No medical, psychological, or clinical services are being provided. We do not diagnose or treat any condition, and we will never tell you to stop taking medication or to stop seeing a doctor, counselor, or other professional.
If you are in immediate danger, call 911. In the U.S. you can call or text 988 to reach the Suicide & Crisis Lifeline at any hour.
2. No guarantees or promised outcomes
You understand and agree that:
- no specific results or outcomes are promised or guaranteed
- each individual responds differently to ministry
- freedom, breakthrough, or change cannot be guaranteed
We minister in faith, and the outcome rests with God. You voluntarily assume all risks associated with participation.
3. Personal responsibility
You acknowledge that:
- you are 18 or older
- you are choosing to participate willingly
- you are responsible for your own physical, emotional, and spiritual well-being
- you will seek licensed professional care when appropriate
A parent or guardian arranges ministry for anyone under 18 directly with us by emailing [email protected], and agrees to these terms on their behalf.
4. Payment policy
Where a payment or love offering is requested for a session:
- full payment is required before booking
- all payments are final and non-refundable
- this includes missed sessions and no-shows
Any exception is at the ministry’s sole discretion and does not change these terms for any other session.
5. Rescheduling policy
- You may reschedule your session one (1) time only.
- Rescheduling must be done at least 96 hours (4 days) before your scheduled session.
To reschedule, use the link in your booking confirmation email or email [email protected].
If you do not reschedule within this window, or ask to reschedule a second time:
- your session is forfeited
- no refund or reschedule is granted
6. No-show policy
Failure to attend your scheduled session, for any reason, results in:
- loss of the session
- no refund
- no rescheduling
7. Chargeback and payment dispute agreement
By signing this agreement, you expressly agree that:
- you will not initiate any chargeback, dispute, or reversal of payment
- you waive your right to dispute this charge through your bank or card provider
Any disputes must be handled directly with the ministry. If you have a concern about a payment, email [email protected] and we will respond in good faith.
You acknowledge that violating this clause may result in:
- collection actions
- legal enforcement of this agreement
If a payment is disputed, we may provide this agreement, the record of your signature, and the payment and booking records to our payment processor and your bank or card provider.
8. Confidentiality and mandatory reporting
All information you share will be treated with respect and discretion. How we store and protect it is explained in our Privacy Policy.
However, you acknowledge that:
- this is not a legally privileged relationship
- we are not bound by therapist–client confidentiality laws
We reserve the right to report, and may be legally obligated to report, disclosures involving:
- murder or intent to harm others
- child abuse or neglect
- elder abuse or neglect
- threats of serious harm or violence
By proceeding, you accept these limitations.
9. Release of liability
To the fullest extent permitted by law, you agree to release, waive, and hold harmless the facilitator, the ministry, and all affiliated parties (including its leaders, ministers, volunteers, and helpers) from any and all:
- claims
- damages
- injuries (emotional, spiritual, or otherwise)
- losses arising from participation
This includes any outcomes before, during, or after the session.
10. Session expectations
You agree to:
- attend the full session (the Bloodline Deliverance Intensive is a 3-hour session)
- be in a quiet, distraction-free environment
- participate honestly and respectfully
11. Technical responsibility
For online sessions, you are responsible for:
- your internet connection
- Zoom access
- being on time and prepared
Technical issues on your end do not qualify for a refund or rescheduling.
12. Governing law and jurisdiction
This agreement is governed by and interpreted in accordance with the laws of the State of Texas, where the ministry is principally operated.
You agree that any legal matters not subject to arbitration will be resolved in a court of competent jurisdiction in the State of Texas.
13. Binding arbitration agreement
You agree that any dispute, claim, or controversy arising out of or relating to this agreement or the services provided will be resolved exclusively through binding arbitration, rather than in court.
- Arbitration will be conducted under the rules of the American Arbitration Association (AAA).
- The arbitration will take place in the State of Texas.
- Each party will bear their own legal fees, unless the law requires otherwise.
By agreeing to arbitration, you waive your right to:
- file a lawsuit in court
- participate in a class action lawsuit
- have disputes decided by a judge or jury
14. Entire agreement
This agreement is the entire understanding between you and the ministry, and supersedes all prior discussions or representations.
If any part of this agreement is found to be unenforceable, the rest of it remains in effect.
The version you sign applies to your request. If we update this agreement, the new version applies only to requests made after the update.
15. Agreement confirmation
By signing, you confirm that:
- you have read and understood this agreement in full
- you agree to all terms stated above
- you are participating voluntarily
You sign electronically by typing your full name and checking the agreement box on the Request Ministry form. Your electronic signature has the same effect as a handwritten one. We keep a record of the name you typed, the date and time, and the exact version of this agreement you signed.
